Effective
We only assessed one quality statement within this domain. People’s needs were assessed before they transferred into the service. Staff had access to the information they needed to deliver people’s care, treatment and support. We received mixed feedback from the home’s partners about the timeliness of people’s referrals to them.

How staff, teams and services work together
People and their relatives raised no concerns about people’s admission into the home and receiving and accessing medical support as required. They told us they were supported to attend appointments and receive ongoing support from relevant health care professionals according to their needs such as regular consultation with the Parkinson's nurse.
The registered manager explained they worked closely with healthcare professionals to ensure people received medical support and treatment in a timely manner. Staff told us they knew people well and escalated any changes in people's well-being to the appropriate healthcare professionals. Staff provided examples of when they requested medical support in emergencies and when they had challenged health care professionals, where necessary, to ensure people received appropriate medical support.
Some healthcare partners told us staff at Nazareth House did not always provide them with the information they required, in a timely way, to help them assess people’s health needs. However, during our onsite inspection, we received feedback from emergency healthcare professionals who told us staff had provided them with the information they required to help them assess the people they had been called to see.
A series of communication systems were used to share information and health care professional recommendations to ensure all relevant staff were aware of people's current support needs. People's care plans, daily records and monitoring charts reflected their current needs and delivery of care. These were monitored by the registered manager and senior staff team to ensure people received timely and appropriate care and treatment. The registered manager was working with the relevant partners to understand and address their concerns about the home’s referral processes.
