Updated 13 February 2024

People received individualised care and support from staff who knew them well. Support plans were informative and provided staff with detailed information on people’s likes, dislikes, personal preferences, care needs and medical history. This enabled staff to support people in the way they wished to be supported; to live full and active lives; develop their independent living skills and maintain relationships with friends and families.

People who were able, told us they were aware of their support plans and had the opportunity to meet with their key worker monthly to discuss and make changes. People chose how they spent their time and were supported by staff to take part in activities and pursue their interests. One person told us they attended a variety of clubs, went shopping for clothes and liked to visit a nail salon to get their nails done. People told us their families and friends could visit, and staff supported them to see their relatives. One person said they were looking forward to going home for Christmas to spend time with their family. Relatives spoke positively about the care and support people received and told us they were actively involved in the planning and development of their loved one’s care and support.

Care plans and risk assessments were regularly reviewed and updated as people's needs changed. Monthly keyworker meetings with people and their relatives provided opportunities for people to talk about their support and make changes.

People’s support plans contained clear information about what each person could do for themselves such as, personal care, choosing their clothes or getting dressed.
