Effective
People’s health, care, and communication needs were assessed. People’s relatives and representatives were involved in decisions when appropriate and people’s consent to their care was sought.

Assessing needs
People’s needs were assessed and regularly reviewed for any changes. People and their representatives had been involved in the care planning process. Person-centred information was available to guide staff about how to support people in line with their views and wishes and health needs. Recognised tools were used in the monitoring and support of people.

Consent to care and treatment
Records of consent were in place and records supported that the relevant people had been consulted in decisions about people’s care.
People’s relatives and representatives confirmed they were involved in decisions about people’s care. Advocates were involved in supporting people to make decisions about their care.
Staff understood the importance of seeking consent from the people they cared for. The role of advocacy services was understood.
