Date of assessment: 9 October 2024 to 5 November 2024. Meadowview is a residential care home providing personal care for up to 41 people. At the time assessment, 39 people lived at the home, most of whom were living with dementia. We conducted a full comprehensive assessment which included 34 quality statements across the 5 key questions of safe, effective, caring, responsive and well led. Our overall rating has changed from good to requires improvement. We found 2 breaches of legal regulations in relation to safe care and treatment, and governance. We have asked the provider to submit an action plan in response to the concerns found. We found concerns regarding medication management and the environment. Some people were not receiving their medication as prescribed, and there was a lack of clear guidance to ensure people received medication as and when they needed them. People were not adequately protected from the risk of fire. Some fire doors were damaged and did not close fully. We also found some fire exits were blocked. We found risks to people were not always assessed or managed well; care plans and risks assessments were sometimes conflicting, inaccurate or incomplete. There was enough staff on duty to meet people’s needs, but some staff raised concerns about staffing levels of an evening. Staff were not always safely recruited. Gaps in employment and medical declarations were not always explored. Governance systems had not been effective in identifying and resolving some of the issues we found during this assessment and there was little evidence existing systems effectively drove improvements. Complaints were not consistently logged or followed up, highlighting gaps in oversight. Staff reported feeling unable to raise concerns or their voices were not heard. home’s culture as negative, stating this was mainly due to management. We did however, observe staff were always kind and caring to people, they respected their dignity and treated them as individuals

People's experience of the service

People and their families expressed positive feedback about the service and the care provided. They reported feeling safe and spoke highly of the staff, highlighting they were treated with dignity and respect. Families felt comfortable raising concerns and were kept informed about any changes to their relatives’ health or needs. Family members were able to visit their relatives with no restrictions and were free to take them out if they wished. Family members felt staff demonstrated a good understanding of their relatives’ needs, were satisfied with the activities available, and felt the care was personalised to meet their relatives’ individual requirements.
