Information was shared between the service and other agencies to ensure continuity of care. All relevant staff were involved in assessing, planning and delivering people's care and support to understand and meet people's needs.

How staff, teams and services work together
People told us they received care from a range of different services, for example, people were supported to access a range of healthcare services and support as needed. A person using the service told us staff supported them to access their local dentist. Another person confirmed staff booked transport to enable them to attend hospital appointments. Relatives told us they were kept up to date about their family members needs and the outcome of health-related appointments. Comments included, “They [staff] tell me when appointments come through and will book transport for me and my family member” and, “If X is not well, they [staff] ring and let me know or if the doctor has seen them.”
The registered manager and staff confirmed they liaised and worked collaboratively with other partner services so as to understand and meet people’s needs. This made sure continuity of care was maintained and people’s needs could be met.
Staff had access to information to assess, plan and deliver people’s care and support. Information was shared between the management team, staff and external partners to ensure continuity of care, for example, when clinical tasks were delegated.
