Our view of the service
Date of Assessment: 15 January 2025 to 30 January 2025. Wood Hill Grange is a care home providing accommodation for nursing and personal care to older people, some of whom are living with dementia. The service can support up to 75 people. At the time of this inspection there were 60 people living at Wood Hill Grange. At the last inspection we looked at safe and well-led and did not provide an overall rating. The service was rated requires improvement in safe and was in breach of regulation 12, medicines management. At this inspection we found improvements were still required in the management of medicines and identified additional breaches in regulations 9 and 17. We found concerns regarding person-centred care, risk management and governance. The service is rated requires improvement overall.
People's experience of this service
We received mixed feedback from people regarding their care and support. People said some staff were very nice while others said staff were rushed and did not speak with them and they did not feel listened to. One person said, “They seem to know me and are quite friendly.” A relative said, “I think they are kind they get on with things as a matter of fact, they are not abrupt.” Another person said, “They [staff] are not looking after me.” And another said, “Staff don’t listen, I raise issues, and they are not resolved.” We observed staff were task orientated and did not always deliver care in a person-centred way. We also observed staff were not always present in communal areas, therefore, people were at potential risk of harm as they were unsupported for periods of time. Staff did not manage identified risk. For example, risks of choking and falling. We saw advice was sought from health care professionals, but this was not always followed. There was a lack of social stimulation and activities were limited. People stayed in their rooms, many said there was nothing else to do. One person said, “I used to go out, I haven’t been out for ages, I only want to go out for 2 hours, but it isn’t possible.” People did not always experience positive outcomes. However, the new management team had identified this and were aware they needed to change the staff culture. They were implementing new ways of working to drive improvements.
