The provider did not have robust processes in place to involve people in assessing and reviewing their health, care and communication needs.

Assessing needs
The provider did not complete an initial assessment of the person's needs with the person and their relatives. The person's communication needs had not been considered and there was no information about how to support or understand their communication.
The provider told us they were using the assessment documentation completed by the local authority as the basis for their care plan. However, the provider was not able to demonstrate they had completed any assessment or review of their own to ensure the information in the care plan remained accurate and provided a detailed and up to date picture of the person's support needs and preferences.
The provider's processes for assessing and reviewing the person's care and support needs were not robust. The person's care plan did not contain a detailed assessment which considered what was important to them including their health, care, wellbeing and communication.
