Dates of assessment were from 5 December 2024 to 17 December 2024. The service is a care home providing support to adults, some of whom are living with dementia. We found 5 breaches of regulations relating to medicines and risk management, consent, person-centred care, governance and fit and proper persons. The governance systems in place had failed to identify and rectify the widespread and significant concerns found at this assessment. Risks to people were not well managed or mitigated and medicines were not managed safely. People were not receiving consistently person-centred care that met their needs. This was mainly due to care plans either not being up to date, or due to them lacking specific details about people’s needs, wishes and preferences. Decisions were made on behalf of people rather than in conjunction with them, with no clear assessment showing these were in the best interest of the person. The provider did not have a safe and effective recruitment process in place. We found the culture of the service was closed with a low staff morale. Staff did not feel they could report concerns openly to the provider and there was a clear divide between some staff and the provider. We have asked the provider for an action plan in response to some of the concerns found at this assessment. In instances where CQC has decided to take civil or criminal enforcement action against a provider, we will publish this information on our website after any representations and/or appeals have been concluded. This service is being placed in special measures. The purpose of special measures is to ensure that services providing inadequate care make significant improvements. Special measures provide a framework within which we use our enforcement powers in response to inadequate care and provide a time frame within which providers must improve the quality of the care they provide.
