During an inspection looking at part of the service

During our announced comprehensive inspection of this practice on 16 April 2018 we found a breach of legal requirements of to the Health and Social Care Act 2008 in relation to:

  • Regulation 19 Fit and Proper Persons Employed

We undertook this focused inspection to check that the provider now met legal requirements. This report only covers our findings in relation to this requirement.

Are services well-led?

We found that this practice was providing well-led care in accordance with the relevant regulations.

Key findings

  • Overall, we found that effective action had been taken to address the shortfall identified at our previous inspection and the provider was now compliant with the regulation.

Background to this inspection

We undertook an announced focused inspection of Bloxham Dental @ Banbury on 16 April 2018. This inspection was carried out to check that improvements to meet legal requirements planned by the practice after our comprehensive inspection on 16 April 2017 had been made.

We inspected the practice against one of the five questions we ask about services: is the service well-led?

During our inspection we spoke with the practice manager and reviewed a range of documentation.

The inspection was carried out by a CQC inspector.

During a routine inspection

We carried out this announced/unannounced inspection on 16/04/2018 under Section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We planned the inspection to check whether the registered provider was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations. The inspection was led by a CQC inspector who was supported by a specialist dental adviser.

To get to the heart of patients’ experiences of care and treatment, we always ask the following five questions:

• Is it safe?

• Is it effective?

• Is it caring?

• Is it responsive to people’s needs?

• Is it well-led?

These questions form the framework for the areas we look at during the inspection.

Our findings were:

Are services safe?

We found that this practice was providing safe care in accordance with the relevant regulations.

Are services effective?

We found that this practice was providing effective care in accordance with the relevant regulations.

Are services caring?

We found that this practice was providing caring services in accordance with the relevant regulations.

Are services responsive?

We found that this practice was providing responsive care in accordance with the relevant regulations.

Are services well-led?

We found that this practice was not providing well-led care in accordance with the relevant regulations.

Background

Bloxham Dental @ Banbury and provides NHS and private treatment to patients of all ages.

The dental team includes 10 dentists, three dental hygienists, two dental hygiene therapists, 17 dental nurses, five trainee dental nurses, nine receptionists and the practice manager. The practice has six treatment rooms.

The practice is owned by an organisation and as a condition of registration must have a person registered with the Care Quality Commission as the registered manager. Registered managers have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated regulations about how the practice is run. The registered manager at Bloxham Dental @ Banbury is the practice manager.

On the day of inspection we collected 25 CQC comment cards filled in by patients and obtained the views of 12 other patients.

During the inspection we spoke with three dentists, three dental nurses, a receptionist and the practice receptionist. We looked at practice policies and procedures and other records about how the service is managed.

The practice is open from 8am to 5pm on Monday and Friday, 8am to 8pm Tuesday to Thursday and 8am to 3pm on Saturday.

Our key findings were:

  • The practice appeared clean and well maintained.
  • The practice had infection control procedures which reflected published guidance.
  • Staff knew how to deal with emergencies.
  • The practice had systems to help them manage risk.
  • The practice had suitable safeguarding processes and staff knew their responsibilities for safeguarding adults and children.
  • Staff treated patients with dignity and respect and took care to protect their privacy and personal information.
  • The practice was providing preventive care and supporting patients to ensure better oral health.
  • The appointment system met patients’ needs.
  • The practice had a culture of continuous improvement.
  • Staff felt involved and supported and worked well as a team.
  • The practice asked staff and patients for feedback about the services they provided.
  • The practice dealt with complaints positively and efficiently.
  • The practice had suitable information governance arrangements.

We identified regulations the provider was not meeting. They must:

  • Ensure recruitment procedures are established and operated effectively to ensure only fit and proper persons are employed.

During a routine inspection

Patient's both long standing and new to the practice all told us that they were very happy with the service they received. They found the staff friendly and helpful. Patient's also told us that they were given clear information of the treatments offered and the costs involved and we saw evidence of this in the patient records.

The staff told us they felt well supported in their roles and were able to gain further qualifications when necessary.

We observed that decontamination procedures were carried out in accordance with national guidelines and all the staff had been appropriately trained and were knowledgeable in their work.

We found that the record keeping was very good and staff were fully aware of the restrictions surrounding disclosure of information and need for confidentiality.

We found that the clinical standards were maintained by regular audits which highlighted any problems which were then acted upon quickly to ensure improvement.
