Effective
People felt involved in managing their health and wellbeing needs. The service shared many positive examples where there had been improvements for people, and they had achieved their goals and outcomes. There was a strong focus on identifying risks to health, with a person focused approach in place. Processes in place helped ensure there was a strong partnership working between people, relatives, staff and health professionals to ensure people’s health and wellbeing needs were met.

Supporting people to live healthier lives
People and relatives felt they were involved in reviewing their health and wellbeing needs. People said they felt listened to and were encouraged to set and achieve goals. One person said, “If we have any health problems, they are dealt with quickly.” Several examples were shared where people were supported to improve their health and wellbeing. For example, one person was told they required palliative care at the service. However, the leaders, staff and partner agencies along with the person began to rebuild their strength taking into consideration, their health and mental health needs. This resulted in the person becoming well enough to move out of the service and back to their home with their family. This success was due to clear, collaborative working. We saw people were actively engaged in things that they were interested in. Where people celebrated a milestone there was a real focus on ensuring recognition and a positive experience, with relatives involved. It was clear this meant a lot to the people. Where people enjoyed their own space, we found that there was a dedicated staff member to also give some time to people to speak with them and give them the emotional support they may need.
The service focused on identifying risks to people’s health and wellbeing early on and on how to support people to help deterioration, but also identified people’s longer term goals health goals and how these could be achieved. There were processes in place to ensure that health assessments, monitoring appointments and checks were completed with relevant health professionals.
Staff felt they knew people well which meant they would recognise changes needed in their support relating to their health care and wellbeing and could respond promptly. They told us there was a proactive approach at the service. Staff gave some examples of how good outcomes had been achieved for people. Professionals we spoke with felt the leaders and staff were extremely person focused and were responsive to people’s needs. One professional said, “The nurse associates are shadowing us, and they are a pleasure to have with you. There are no challenges. They are proactive with contacting us and building positive relationships.”
