Equity in access
People and relatives told us they had the care they needed and access to any support. This helped promote positive health outcomes and equity of access to health provision.
Staff had received key training in areas such as equality and diversity. Care records we reviewed showed people were able to access external services promptly particularly healthcare services and professionals such as the GP and dieticians or district nursing teams. Regular multi disciplinary meetings were held by the management team and staff to constantly review people's changing needs and ensure referrals to external services were made. The actions arising from those referrals and assessments were monitored within those reviews and changes acted on accordingly. These included areas such as weight management, pressure care, risk of falls and mobility concerns.
