People were assessed before they used the service to ensure their needs and preferences were identified and could be met. People were supported to understand their right to consent and empowered to make their own decisions when involved in planning how their care and support was provided. Staff understood the importance of ensuring people understood what they were consenting to and obtaining people’s consent before they delivered care.

People’s relative’s and health professionals were consulted with when needs were assessed. A relative told us they were asked to provide information to support the assessment. They confirmed staff listened to their feedback and this was incorporated into the care plan.

Staff and leaders within the service were fully aware of the processes to follow when supporting people to make decisions. Staff completed appropriate training in mental capacity and this area was regularly discussed during staff meetings.

People’s care and social needs were assessed in a person-centred way to ensure staff were able to support the individual appropriately. Assessments included information around people’s healthcare needs, preferred communication methods and how to best support the individual. Care was delivered in line with national guidance and legislation and we saw this from care records and staff meetings we reviewed.

We observed staff ask people for their permission before caring out tasks to support them. People were seen to be comfortable and relaxed when having their needs attended to.

Staff told us they assessed people to see if they had capacity to make decisions, and would not assume people did not have capacity. Where people did not have capacity, this was recorded and appropriate decisions made in people’s best interest. One member of staff said, “People have enough opportunities to make choices. We have residents’ meetings and feedback forms tailored to each individual. For some it is just happy/sad cards. All decisions are recorded.” 2 people told us they have advocates to support them when needed in relation to decisions. An advocate is someone who can offer support to enable a person to express their views and concerns, access information and advice, explore choices and options and defend and promote their rights.

There were effective systems in place to follow decision making principles on behalf of people who lacked capacity. Staff received training in the mental capacity act and demonstrated a good understanding of supporting people to make informed decisions. Where people living at the service were unable to make their own decisions, records of best interest meetings and specific decisions were in place.
