Our view of the service

Blossom HCG is a domiciliary care service providing personal care for up to 16 people living with a learning disability and / or autism in supported living environments. We expect health and social care providers to guarantee people with a learning disability and autistic people respect, equality, dignity, choices and independence and good access to local communities that most people take for granted. 'Right support, right care, right culture' is the guidance CQC follows to make assessments and judgements about services supporting people with a learning disability and autistic people and providers must have regard to it. This assessment has been completed following the Care Quality Commission (CQC) new approach to assessment; Single Assessment Framework (SAF). Our assessment started unannounced with our on site assessment on 14 February 2024. We assessed 29 quality statements from the safe, caring, responsive and well-led key questions and found areas of good practice and concerns. The scores for these areas have been combined with scores based on the key question ratings from the last inspection. This assessment did not cover all parts of our Single Assessment Framework. People were not always supported to have maximum choice and control of their lives and staff did not support them in the least restrictive way possible and in their best interests; the policies and systems in the service did not support this practice. Incidents that placed people at risk of harm were not robustly investigated or reported. Management did not promote an open and reflective approach to learning from incidents when things went wrong. The providers systems for monitoring the quality of care and safety matters had failed to identify concerns found during this assessment. People and those important to them were involved in planning their support, but not always in decisions that were in people’s best interests. We have found breaches in relation to safeguarding and governance.

People's experience of this service

We observed staff supporting people in a kind and respectful way. Relative felt that the staff were responsive to their health and wellbeing needs and gave people opportunities to reach their goals and aspirations. People were not always kept safe from harm or supported in the least restrictive way to mote their choice and independence.
