Care plans were out of date and did not accurately reflect the care people received. One person said, 'I have a care plan but it is really out of date.' Another person told us their care needs had changes significantly but their care plan had not been updated to reflect the changes. Records we looked at were out of date and did not reflect the care delivered.
Staff told us they had not received their supervision sessions. The manager told us these should occur every two months according to the provider's policy. Records showed half of the staff had not had a supervision or appraisal meeting with their manager in the last three months.
