Linden Manor is a residential care home providing accommodation for persons who require nursing or personal care to up to 28 people. The service provides support to older people, people with dementia, people with physical disabilities and people with sensory impairment. At the time of our inspection there were 26 people using the service. Linden Manor is a care home without nursing care. CQC regulates both the premises, and the care provided, and both were looked at during this inspection. The last rating for this service was requires improvement in the Key areas of Safe, and Well-Led (published 18th January 2024). The service has been rated requires improvement for the last 4 consecutive inspections. We reviewed information we had received about the service since the last inspection. We sought feedback from the local authority and professionals who work with the service. We inspected the areas of Safe, and Well-Led. This inspection was carried out by 2 inspectors, with support from a 3rd.We carried out unannounced site visits on the 17th of June 2024, and the 12th of July 2024, and finished the assessment on 29th July 2024. We saw that improvements had been made from the findings from the last inspection. However further improvements were required around ensuring best practice with best interests, and mental capacity assessments. Not all risk assessments were completed fully and accurately around health needs. Assessments needed to consider people’s long-term independence, be consistent in the information they contain, and follow the provider’s care planning policies and procedures. We found two breaches of the legal regulations in relation to safe care and treatment and governance. Staff did not always assess risks to people's health and safety or mitigate them where identified. People did not always have care plans to guide safe practice. Governance systems and audits were not effective in identifying or addressing areas for improvement.
