Assessing needs
We did not receive any patient feedback specific to this quality statement.
Staff and leaders told us they had processes to ensure that risks to people’s health and wellbeing identified early and to prevent deterioration. However, our clinical searches showed some of these processes were not working as intended.
Our clinical searches showed that processes didn’t always work as intended. For example, the health needs for people at risk of diabetes and were not always identified early so that deterioration could be prevented.
Delivering evidence-based care and treatment
We did not receive any patient feedback specific to this quality statement.
Staff and leaders told us they had processes to ensure that they kept up to date with legislation and current evidence-based practice. However, our clinical searches showed some of these processes were not working as intended.
However, our clinical searches showed that patients health was not always monitored in line with current evidence based practice and guidelines. For example those taking medicines that required monitoring and those with long-term conditions.
