During an annual regulatory review

We reviewed the information available to us about Bere Regis Surgery on 2 October 2019. We did not find evidence of significant changes to the quality of service being provided since the last inspection. As a result, we decided not to inspect the surgery at this time. We will continue to monitor this information about this service throughout the year and may inspect the surgery when we see evidence of potential changes.

During a routine inspection

Letter from the Chief Inspector of General Practice

We carried out an announced comprehensive inspection at Bere Regis Surgery on 18 August 2016. Overall the practice is rated as good.

Our key findings across all the areas we inspected were as follows:

  • There was an open and transparent approach to safety and an effective system in place for reporting and recording significant events.
  • Risks to patients were assessed and well managed.
  • Staff assessed patients’ needs and delivered care in line with current evidence based guidance. Staff had been trained to provide them with the skills, knowledge and experience to deliver effective care and treatment.
  • Patients said they were treated with compassion, dignity and respect and they were involved in their care and decisions about their treatment.
  • Information about services and how to complain was available and easy to understand. Improvements were made to the quality of care as a result of complaints and concerns.
  • Patients said they found it easy to make an appointment with a named GP and there was continuity of care, with urgent appointments available the same day.
  • The practice had good facilities and was well equipped to treat patients and meet their needs.
  • There was a clear leadership structure and staff felt supported by management. The practice proactively sought feedback from staff and patients, which it acted on.
  • The provider was aware of and complied with the requirements of the duty of candour.

We saw one area of outstanding practice:

The practice healthcare assistant (HCA) had identified 383 patients over the age of 75 years that were vulnerable and to date had carried out 170 assessments on the most vulnerable and isolated. These assessments were an evolving process and included identifying need in housing, physical health, mental health and social need and associated risks such as risk of falls or poor nutrition using a nationally recognised tool. The HCA liaised with dentists, opticians, housing departments and the practice GPs where areas of concern were identified.  The practice also worked closely with local organisations to provide social events and lunch clubs. This initiative had reduced the hospital admissions for this age group by 33%.

During a routine inspection

The Bere Regis Surgery is a GP practice that offers general medical care to approximately 3500 patients in the Bere Regis and the surrounding rural area.

We visited the practice, which is based at Manor Farm Road, Bere Regis, Dorset on 3 June 2014 to carry out our inspection.

Patients provided feedback (before and during our inspection) and told us they were happy with the care and treatment they received and they felt safe. They told us staff treated them respectfully and were helpful and added that their treatment was clearly explained to them and they were able to make choices.

Staff spoke politely to patients and consultations were carried out in private treatment rooms. Information was available for patients which included health promotion, access to support services and information about the practice and the services it provided.

We found that there were some systems in place to manage risk to patients but were not always monitored. These included recruitment and disclosure and barring service (DBS) checking procedures. We also found that a number of risk assessments had not been carried out. These included health and safety and fire safety. Improvements were needed in these areas to protect patients from harm.

During our inspection we looked at how well services are provided for specific groups of people and what good care looks like for them. The population groups we reviewed were:

  • Older people
  • People with long-term conditions
  • Mothers, babies, children and young people
  • The working-age population and those recently retired
  • People in vulnerable circumstances who may have poor access to primary care
  • People experiencing a mental health problems

We found the practice provided a responsive service for some patients within each population group.
