About the service

Springfield Manor Gardens is a care home providing nursing and personal care for up to 58 people at the time of the inspection there were 55 people living in the home. The home is divided into three units, which includes a nursing and residential units, on each of the three floors. There are a variety of spacious communal areas and external grounds.

People’s experience of using this service and what we found

People were supported to stay safe and manage the risks in their lives. Some environmental risks had been identified during the inspection and had been addressed promptly by the provider.

People living in the home had mixed views about their experiences. Some people found it difficult to communicate with staff due to language difficulties.

People were supported by enough trained staff. Staff felt they had received enough training and had enough information about people's needs to support them effectively.

Staff were seen to be polite, respectful and kind to people.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

The service was well-led. Staff had mixed experiences about the management of the home. This was in the process of being addressed.

The last rating for this service was good (published 22 May 2021).

Why we inspected

We received concerns in relation to; the management of medicines, staffing and the management of people’s nursing care needs. As a result, we undertook a focused inspection to review the key questions of Safe and Well-Led only.

We reviewed the information we held about the service. No areas of concern were identified in the other key questions. We therefore did not inspect them. Ratings from previous comprehensive inspections for those key questions were used in calculating the overall rating at this inspection.

We have found evidence that the provider needs to make improvements. Please see the safe and well-led sections of this full report.

We looked at infection prevention and control measures under the Safe key question. We look at this in all care home inspections even if no concerns or risks have been identified. This is to provide assurance that the service can respond to COVID-19 and other infection outbreaks effectively.

We will continue to monitor information we receive about the service until we return to visit as per our re-inspection programme. If we receive any concerning information we may inspect sooner.

About the service

Springfield Manor Gardens is a nursing home providing personal care to 54 people at the time of the inspection. The service is registered to support up to 58 people within three units on three floors.

People’s experience of using this service and what we found

Documentation related to one person’s medicine did not reflect their prescription. We have made a recommendation about this. Staff were recruited safely. Safeguarding procedures were in place to

protect people from the risk of abuse by staff who understood and were trained on how to recognise and respond to concerns. Infection control systems and audits continued to ensure a clean environment.

The registered manager ensured staff received training that met people's needs. Feedback showed most people were happy with meals and quantity of food provided throughout the day. One person said, "The food is good." People received support with their healthcare needs. The environment was maintained to meet people’s needs.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

The management team were clear about their roles and responsibilities. They worked in partnership with a variety of agencies to ensure people's health and social needs were met. The registered manager used a variety of methods to assess and monitor the quality of the service. This ensured the service continued to be monitored and improve.

Rating at last inspection and update: The last rating for this service was requires improvement (published 11 December 2019) and there were multiple breaches of regulation. The provider completed an action plan after the last inspection to show what they would do and by when to improve. At this inspection we found improvements had been made and the provider was no longer in breach of regulations.

Why we inspected

We received concerns in relation to recruitment, staffing and governance. As a result, we undertook a focused inspection to review the key questions of safe, effective and well-led only.

During an inspection looking at part of the service

Springfield Manor Gardens is a nursing home providing personal care to 34 people aged 65 and over at the time of the inspection. The service is registered to support up to 58 people.

We found the following examples of good practice.

• Visiting the home was restricted. All visitors were temperature checked before admission. Visitors were required to follow best practice guidance and wear appropriate PPE. The visiting policy was discussed with people, their families and friends.

• People received support to maintain their relationships with family and friends. This was achieved through telephone calls and video calls. The provider supported compassionate visits for people who were at end of life. The provider had created a newsletter to share with family members what they were doing to minimise risk and keep people safe. Work was taking place to provide onsite visits for relatives.

• Admissions into Springfield Manor Gardens were risk assessed and followed government guidance. Covid-19 care plans had been completed to meet people’s needs during the pandemic.

• Whole home testing for Covid-19 for all people and staff had occurred and was ongoing on a regular basis. The manager had processes to implement to keep everyone safe should a positive test occur. Easy read literature was available to people so they may be able to easier understand the restrictions in place.

• Staff said the management team were very aware of what precautions needed to be in place and was very protective of people and staff. We saw up to date infection prevention and control policies that contained current guidance.

• All staff had received training on how to wear, when to wear and how to dispose of PPE.

• We observed staff wearing PPE appropriately. Staff were observed socially distancing while spending time with the people and engaging in activities. Staff had designated areas to change into and out of their uniforms.

• We observed Springfield Manor Gardens looked clean and hygienic. Housekeeping staff followed a cleaning schedule that included regular cleaning of high touch areas such as door handles and switches.

During a routine inspection

About the service

Springfield Manor Gardens is a nursing home providing personal care to 32 people aged 65 and over at the time of the inspection. The service is registered to support up to 58 people.

People’s experience of using this service and what we found

People told us they received their medicines as they should. However, the provider did not have

robust systems for the proper and safe management of medicines. The provider did not consistently act in accordance with the requirements of the Mental Capacity Act 2005 and associated code of practice. Systems and processes did not always assess, monitor and improve the service provided.

We looked at systems for the recruitment of staff and staff deployment. We have made a recommendation about staffing and a recommendation on the deployment of staff.

People's care and support had been planned in partnership with them and their relatives. Staff had received regular training and supervision to support them in their roles. People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests. The policies and systems in the service did not always support this practice.

People were protected from the risk of abuse and avoidable harm by staff who understood how to recognise and respond to concerns. People told us they felt safe when supported by contracted staff. Relatives told us they felt their family members were safe at Springfield Manor Gardens. Risk assessments had been developed to minimise the potential risk of harm to people during the delivery of their care.

People received person-centred care which was responsive to their needs. People's communication needs had been assessed and where support was required these had been met. The manager dealt with people's concerns and complaints appropriately.

People were positive about the service and said staff were kind and caring. People were treated with dignity and respect and their right to privacy was upheld. The service could provide people with information about local advocacy services, to ensure they could access support to express their views if they needed to. The management team worked with people’s advocates.

The service worked with a variety of agencies to ensure people received all the support they needed. People were happy with the new provider and how the service was managed. Staff felt well supported by the management team. The provider co-operated throughout the inspection and acted on concerns raised.

Rating at last inspection

The last rating for this service was requires improvement (published 10 October 2018). Since this rating was awarded the registered provider of the service has changed. We have used the previous rating and enforcement action taken to inform our planning and decisions about the rating at this inspection.

Why we inspected

The inspection was prompted in part due to concerns received about the management of the service. A decision was made for us to inspect and examine those risks.

Enforcement

We have identified breaches in relation to the management and administration of medicines, consent and working in accordance with the Mental Capacity Act 2005 and the management and oversight of the service delivered.
