Date of assessment: 18 December 2024 to 13 January 2025. Oaken Holt Nursing and Residential home is a residential care home providing the regulated activity of accommodation for person's who require nursing or personal care for up to 60 people. The service is registered for Treatment of Disease, Disorder or Injury (TTDI) however they did not provide nursing care at the time of assessment. The service provides support to older people some of whom are living with dementia. At the time of our assessment there were 23 people using the service. The assessment was prompted due to concerns received about staffing levels, management of medicines, governance and safeguarding concerns. A decision was made for us to assess and examine those risks. We looked at 15 quality statements across 2 key questions. We found 4 new breaches of regulation in relation to safe care and treatment, safeguarding, good governance, and notifications. People’s care plans did not always have clear, consistent and up to date information to enable staff to support people appropriately. Risks to people’s safety were not always managed effectively, and documentation including assessments were not always reflective of peoples risks and support needs. Safe recruitment processes were in place, however, the service had struggled with recruitment of staff, as a result staffing was supplemented by a large number of agency staff. People received their medicines on time and in a safe way. For people who were prescribed PRN (as and when required) medicines, protocols were in place to ensure people received these appropriately. Where people missed medicines, documentation did not always evidence what action was taken. Although newly implemented governance systems were in place, these had not yet been embedded into practice and had not identified some of the shortfalls found during the assessment.
