Well-led
We identified two breaches of the legal regulations. There were not established and effective governance systems operating to provide timely and effective oversight of risks. For example, people’s tests results were not reviewed and acted upon in a timely manner. The prescribing and monitoring of medicines were not overseen, and poor prescribing practice placed people at risk of harm. Staff were not trained and/or supervised to triage patients safely and some staff operated outside their skills and knowledge. Staff had not learnt from previous concerns of significant events resulting in them being repeated. Some staff failed to adhere to professional standards of conduct operating contrary to their professional registration by treating parties known to them.

Governance, management and sustainability
The provider did not have established and effective governance, management and accountability arrangements. Lead clinicians did not oversee risks such as safeguarding, medicine management and clinical practice. The provider did not conduct checks on individuals or collective prescribing behaviours to ensure adherence to safe practice. Non-clinical staff were appointed responsibilities for reviewing clinical data without training, support, or oversight. The provider did not meet with staff individually or collectively to assess and review performance. Significant events were not learnt from so similar incidents reoccurred. Concerns raised in clinical meetings were not actioned and resolved.
