Condition of this registration relating to carrying out this regulated activity
The registered provider must not admit any service user to The Willow, 19 Oakfield Street, Altrincham, WA15 8HQ without the prior written agreement of the Care Quality Commission. This extends to and includes the re-admission of any service user who has been resident at the home at any time and who may be seeking to return, as well as the admission of any service user from any other registered location that is operated by the provider.
In the event of a proposed admission to the service, the registered provider must submit written information which evidences how they propose to deliver safe care and treatment to that service user. The information provided should include, but is not exclusive to;
a. A full admission assessment, including an overview of how the registered provider will be able to meet the needs of the Service User.
b. Initial risk assessments, risk management plans, care plans and positive behaviour support plans which demonstrate that there are plans in place to mitigate identified risks as much as practicably possible.
c. Evidence that there will be sufficient numbers of adequately trained staff to provide care and treatment to the Service User.
d. Evidence that the premises and equipment at The Willow has been risk assessed and is suitable to provide safe care and treatment to the Service User.
e. Information of how the registered provider propose to monitor the care and treatment that is delivered to the Service User in a way that would identify areas for improvement when needed.
The registered provider must, by the 26 September 2023, provide a report to the Commission setting out the actions taken to implement a safe and effective system for the assessment and management of admissions of new or returning service users to The Willow, 19 Oakfield Street, Altrincham, WA15 8HQ.

The registered provider must only accommodate a maximum of 1 service user at The Willow.
