Our view of the service

Date of assessment 11 to 17 April 2024. The Larches- Tiverton is a residential care home providing personal care to up to 20 people. The service provides support to people aged 65 and over with a physical disability or dementia. The assessment was triggered by people and other healthcare professionals contacting the Care Quality Commission (CQC) to raise concerns regarding staffing, financial viability, infection control and poor practice. We looked at 14 quality statements; Safeguarding; Involving people to manage risks; Safe environments; Safe and effective staffing; Infection prevention and control; Medicines optimisation; Independence, choice and control; Equity in experiences and outcomes; Capable compassionate and inclusive leaders; Governance and assurance; Freedom to speak up; Shared direction and culture and Learning, improvement and innovation. During this assessment we identified breaches in relation to safe care and treatment, fit and proper persons employed, complaints and good governance. People were not consistently protected from unsafe care or treatment or the risks associated with medicines. The provider did not operate an effective complaints procedure and staff were not always employed safely in line with requirements. The provider did not have effective governance arrangements. We have asked the provider for an action plan in response to the concerns found at this assessment.

People's experience of this service

People told us they felt safe, knew staff well and felt confident to raise any concerns. Observations during the assessment showed that staff knew people well and we saw several positive interactions. People felt the service was clean and staff practice relating to infection control was positive. Positive feedback was received in relation to the competency of the staff that supported them and no significant concerns were raised about the amount of staff on duty. However, we found that people’s experience, or those acting on their behalf, could be improved in relation to planning care and support.
