Effective
Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. At this inspection we rated this key question as good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this. People had their needs assessed and their choices listened to. People’s needs assessments formed the basis of their care plans and these were accurate and up to date. People’s care needs were regularly reviewed and any changes or updates that were required were made accordingly. People were supported to understand the care and support that was being offered to them. We did not assess all the quality statements within this key question, as we did not identify concerns relating to those areas which we judged as being met at our last inspection.

Assessing needs
People received care and support that was planned and delivered in line with their assessed needs and wishes. People told us their needs and wishes were assessed before they received support and their care plan was based on these assessments. People also said they were central to the pre-admission assessment process. Comments included, “The care plan is in the folder kept in my [family member’s] home, they [staff] keep it updated and it is reviewed with us,” and “We were involved in the process. The care plan is informative, setting out how our care and support will be provided.”
Staff told us they were able to access people’s care plans via a folder kept in the person’s home and also via an app on their mobile telephones. They said this provided them with the information they needed to support people. Comments included, “We can access people’s care plans via the folder in people’s homes and also via an app on our phones.”
People's assessed dependency needs and wishes were used to help the provider develop personalised care plans for everyone they supported. There were appropriate policies and procedures in place to do with risk assessments and care planning for staff to follow. We saw risk assessments for people needing support and care with these needs. Where risks were identified, care plans covered how these risks would be supported.

Supporting people to live healthier lives
People using the service told us they had access to relevant external health care professionals and agencies as and when required including, local GP services and community nurses. They told us that when necessary their carers liaised with health care professionals. People and their relatives said this helped them to keep healthy.
Staff told us they supported people to live healthier lives. They said people’s care plans included their personal health objectives and received emergency first aid training as part of their induction.
People were supported to stay healthy and well. Care plans detailed people’s health care needs and conditions and the action staff needed to take to keep people fit and well. Staff ensured people routinely attended scheduled health care appointments and had regular check-ups with a range of external, community-based medical and health care professionals.
