Care provision, Integration and continuity

Score: 3

The service understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. Most relatives confirmed this, and one relative told us, “The provider had a pathway meeting for my relative with the social worker and personal advisor present. They have also been in communication with the learning disability team in the area my relative previously lived in, who had been involved with them.” Staff had completed training and understood the Equality Act. The registered manager told us, they have regular MDT meetings which they encouraged people to attend, and if they did not want to, their views could still be shared with their permission.
