Involving people to manage risks
People told us that they felt involved in planning their care and support. One person said, “Yes we have discussions, on what is good and right for [Name]”. Risks to people were assessed and collaboratively managed with people, their loved ones, the staff team and other professionals. There were risk assessments in place for various aspects of people’s care such as the risk of risk of falls and environmental hazards. When we asked people and relatives if they were involved, they told us that this was very important and they were involved in making decisions especially around their care and support.
Care staff and leaders were very knowledgeable about individual’s care and support needs and told us about peoples care plans in detail and knew people well. The staff we spoke with understood people’s risks and how to support them to reduce these. Staff told us they knew where to find information in the digital care app about risks and were knowledgeable about how they would record any issues or concerns and leaders had good knowledge of each person.
The registered manager, area managers and nominated individual spoke with us about care planning and how they involved people to manage risks. They had conducted a survey with people and their relatives which showed that most people were happy with the level of involvement in their care planning. We reviewed risk assessments as part of people’s care plans which described actions for staff to take to support people to manage risk and keep them safe.
