We assessed 1 quality statement in the effective key question and found areas of good practice. The scores for these areas have been combined with scores based on the rating from the last inspection, which was good. Our rating for this key question remains good. Care records evidenced people's needs were assessed prior to moving into the home with their involvement or those acting on their behalf. Care and risk management plans were developed around the initial assessment of people’s needs. Areas of need, how they were to be met and the expected outcomes were clearly identified within people’s individual care plans. People’s choices and wishes were fully considered throughout the assessment and care planning processes. Regular reviews of people’s care took place to make sure they continued to accurately reflect their needs and choices.

Assessing needs

People's Experience Family members who spoke with on behalf of people told us their relatives needs were assessed and kept under review. They also told us they were involved in the assessment and care planning process. Their comments included, “Yes fully involved in [relatives] care plans we go through and review and update every 12 months, feel most definitely [relatives] needs are fully met” and “Yes involved in care plan and done a lot of discussion around arranging care plan when [relative] first moved to Paisley, regular reviews and any changes even the littlest ones are updated in the care plan and I am told.”

Staff told how they are provided with information about new people. One staff member said, “We just try to spend time getting to know people there is information available for us to read.” Another staff member told us they would love to have more time to spend reading up about people before they came to live at the home. Staff were able to describe the needs of the people they supported very well. Managers told us people’s needs were assessed before they came to live at the home and kept under review.

People's needs were assessed and kept under review. Care plans clearly set out people's needs and how they were to be met. Records were in place and completed for aspects of people's care which required observation and monitoring including food and fluid intake, repositioning and skin integrity. Staff were kept updated about people’s changing needs through handover meetings held at the beginning of each shift.
