Overall, the service was effective. Where people had needs relating to their health these were understood and staff helped them to monitor and manage their conditions. However, some improvements were needed to ensure people were consistently involved in assessments relating to their health, wellbeing, and care.

Assessing needs

People had mixed views about how they had been involved in their assessments. Some people felt they were not really involved in their care and support. One person told us, "They don’t really talk to me about my care and what I need to do to help myself." Whilst other people told us they felt engaged in their care, with 1 person telling us, "They go through my care plan with me. I’m very involved in my care."

The registered manager told us people had their needs assessed and care plans put in place to guide staff on how to support people. They described using a range of tools to assess people's needs and how these were documented in the electronic records system to give guidance for staff on how to meet people's needs and minimise risks. Staff told us they mostly had guidance in place to support them with meeting peoples assessed needs, however staff did comment they were unaware of how to support someone with distress and anxiety.

Assessments and care plans did not consistently assess all known needs for people. For example, 1 person had known needs relating to distress and anxiety. This had not been assessed and there was no care plan in place to guide staff on how to support the person. However, where needs had been assessed for other aspects of peoples care there were clear plans in place to meet those needs and evidence of regular reviews.

Supporting people to live healthier lives

People told us they had support to manage, maintain and improve their health. People described having referrals to a range of different health practitioners. One person said, "An Occupational Therapist and a Physiotherapist visit me here to help get me moving." Some people shared examples of how staff supported them to monitor and maintain their health, with 1 person saying, "They check my pulse, blood pressure and temperature every day to check I don’t have any infections." Some people described improvements to their health and wellbeing since being admitted to the home. One person told us, "The staff have nursed me back to health." Another person said, "They used to have to use the hoist to get me out of bed, but I don’t need that anymore, I’ve improved lots whilst I’ve been here, and I am ready to go home now."

The registered manager told us there were systems in place to monitor people’s care. They described how people’s individual health needs were monitored, for example using charts to record peoples’ fluid intake and how any concerns would be escalated to a relevant health professional. Staff told us they understood how to support people to maintain their health and wellbeing. Staff told us they were kept up to date about people's health needs and understood how to escalate any concerns they had to seek input from others to support people with their health needs.

Assessments and care plans were in place to consider peoples individual health needs and guide staff on how to meet them. Records showed staff knew what to look for and when to escalate concerns. There were plans in place to ensure monitoring information was captured in people's care records and where needed we could see staff had engaged other health professionals such as Doctors, Physiotherapists and Occupational Therapists.
