We found that audits and processes in place to identify and mitigate risks, including analysing incidents, were not always effective. During our assessment of this key question, we found concerns around provider audits, oversight and leadership of the service which resulted in a breach of Regulation 17 Good governance (1) (2) (a) (b) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. There were policies in place which were regularly updated to ensure they remained comprehensive.

Staff felt included in the service and told us they felt supported in their roles. They were positive about the registered manager and leadership team.

Staff practices were underpinned by policies and procedures which were regularly updated to ensure they remained comprehensive. The processes in place ensured staff were given support to enhance their training and develop within their roles.

Governance, management and sustainability

Staff were clear about their role, responsibilities and accountability and that of others within the organisation. Staff were positive about working for the provider and the training, support, communication systems in pace. One member of staff told us, "We can raise concerns, no problem. The manager has an open door policy. Very supportive. I can go to her about anything. She acts swiftly.”

Provider audits and processes were not always effective in identifying issues and put people at an increased risk of harm. Incident management was not comprehensive and lacked managerial oversight.
