We identified 1 breach of the legal regulations in relation to meeting people’s nutritional and hydration needs. People had not had their nutritional needs adequately assessed and the provider failed to provide people with food that met their needs and choices.

Delivering evidence-based care and treatment
Although people’s religious and cultural needs were identified, these needs were not always met. We received mixed feedback about the quality of food provided to people. Some people told us they did not receive food that met their cultural needs and choices, one person who required a fortified diet said they preferred hot food, and the food they were served was cold. Other people were happy with the food they received and told us they were given a choice of meals, if they did not like what was on the menu an alternative was provided. Records showed that staff did not always follow people’s nutrition care plans. One person’s care plan stated they required a pureed diet. However, the person’s care records referred to them being given both a soft and pureed diet. Staff had also recorded in the daily notes the person had eaten toast.
Feedback from staff was mixed; some staff said people were offered food that met their cultural needs and choices, other staff said this happened rarely. Staff were aware of people’s dietary needs.
People had not had their nutritional needs adequately assessed, as nutritional assessments were not fully completed or were incorrect. We saw that one person’s nutritional assessment had not been completed with information about their weight or medical conditions that may affect nutrition. This person was at high risk of malnutrition. The provider failed to provide a variety of nutritious, appetising food to meet people’s needs. The provider’s ‘nutrition in care homes’ policy stated, religious, personal or cultural special needs would be fully catered for. It also stated menus would offer a choice of foods and sample menus would be referred to an external dietician monthly. This policy was not followed in practice. Menus in place at the service did not include a variety of nutritious food options and much of the food on the menu was processed, including hot dogs, sausage rolls and ravioli. On four occasions in June 2024 there was only one option for lunch on the menu. Following the assessment, menus were reviewed and amended to provide people with a variety of food choices. There were several documents being used to provide oversight of the dietary needs of people in the home. The information contained in these documents was inconsistent and did not correlate with the information contained in people’s care plans.
