Effective
We found that there were differences in how individuals' needs were evaluated when they first joined the service. Some people had assessments before they started using the service, while others did not. This meant in some cases, there wasn't enough information to provide safe care. Ongoing assessments did not always contain enough information to support people safely. Some people didn't know about their care plans or how they were developed, while others told us they had regular updates and communication from staff.

Assessing needs
There were no clear processes in place to assess people's needs when they joined the service, leading to inconsistencies. Some people had an assessment completed prior to using the service, while others did not. The registered manager told us they were given to people to complete and to then hand back to the provider. We found one person had been using the service for several months and their assessment had not yet been returned. This meant the provider could not be assured they had all relevant information about the person and their care needs to deliver care safely. Ongoing assessments, for example, moving and positioning assessments were not always in place and did not contain sufficient information to keep people safe. We received mixed feedback from people in this area. Some people told us they had never had sight of their care plan and were not always fully involved in its development. Whilst others told us they had a Care Plan which the carers updated electronically via an App. One person said, “The carers always keep in touch and the communication is good.”
