Responsive
Care planning was person-centred. Whilst governance regarding care records and risk assessments needed to improve, the service ensured staff were able to act on people’s changing care needs. Staff acted proactively when needed. People were supported to access care and information in ways that met their personal preferences and needs.

Equity in experiences and outcomes
People raised no concerns with regard to inequalities of experience when accessing health and social care. The majority of people we spoke with either arranged their own health appointments or had family support to do so. They confirmed staff were helpful if needed and we saw evidence of staff highlighting the need for people’s care needs to be reviewed when their needs had changed. They then ensured the appropriate health and social care professionals (for instance, nursing, chiropody) were involved to ensure people experienced good outcomes.
