People were supported to access health services including health screening and specialist health services. People and their relatives spoke positively of the support provided by staff and the management team to monitor and improve their health. This included providing information and support to enable them to access and understand their health care appointments. Staff had a good understanding of people’s needs and were knowledgeable as to their role in promoting people’s health and wellbeing. There was effective partnership working between external health and social care professionals, and the management and staff teams. People’s health care needs were clearly documented within their support plans, and records monitoring their health were in place to encourage and promote health and wellbeing. Information was provided in an accessible format to support people in understanding their health care appointments and needs.

Supporting people to live healthier lives

People were supported to access health services including health screening and specialist health services. A person told us how staff supported them to attend health appointments. Relatives praised the staff and management team, in how they responded to their relatives health needs, which included an account as to how their relatives health and wellbeing had improved. A relative said, “The staff take my relative to appointments. They show my relative photographs about what is going to happen.” Another relative told us their relative had complex health issues and with the support of staff and the actions of the management team, they were accessing the services they needed.

Staff demonstrated a good understanding of people’s health care and support needs. Staff provided a clear account of the support they provided to encourage people to access health care services and of their role in how health care needs were monitored. A staff member told us, “We support [person’s name] to health appointments, and we do this by using social stories up to 2 weeks before to help prepare them.”

Partnership working with a range of health and social care professionals supported people’s health and welfare. People’s support needs were documented and regularly reviewed in response to any changes to their care and support needs. Health action plans were used to record people’s health needs, including appointments. Records detailed the date of the appointment, the reason for the visit and the outcome. Hospital booklets were in place which provided important information to be shared with other agencies such as ambulance and hospital staff. This supported people with their ongoing care in the event they attended hospital. Information was available for people, which included social stories incorporating easy read or pictorial reference to support in the preparation of upcoming health appointments, such as the dentist, optician or for blood tests. People’s health needs were further supported by plans which included the importance of going for walks, jumping on the trampoline and fitness support sessions.
