This key question has been rated good. We reviewed 3 quality statements for this key question. We received mixed feedback from people and their relatives in relation to how they were involved in their care planning. Some people had not been involved whilst others had. Leaders told us initial assessments were carried out however, some people told us further reviews of their care had not been completed with them.

Assessing needs

People told us when support commenced from Be Kind Care Ltd a meeting was held to assess people’s needs and wishes. People told us that further reviews of their care plans had not taken place.

Leaders told us people had an initial assessment undertaken when support commenced and that this was reviewed as required, however no evidence of this was seen. Staff told us some care plans were in place on the app, however when asked to see these, the staff members were unable to access the app.

Not everyone had a copy of their care plan in their homes, some people who did have these in place had information that was out of date or information missing.

How staff, teams and services work together

People told us some staff liaised well with them and their relatives, however this was not the case for all. Some people said they needed to repeat information and did not feel this was actioned or taken on board. For example, closing the bedroom window before leaving one person’s house.

Leaders told us staff provided feedback to them about changes to people's needs and the care plan was updated. However, upon checking care plans this was not evidenced. We saw evidence of team meetings taking place, however these lacked detail to ensure staff members had the knowledge and skills to undertake their roles.

Monitoring and improving outcomes

We received mixed feedback from people and their relatives regarding care planning, with some stating they had been involved in initial assessments and care plan writing when commencing support and others stating that this was not always the case. There was no evidence of care plans being reviewed or risk assessments being in place for people as required.

Leaders told us information was communicated between staff and leaders regarding changes in people’s needs, however we found no evidence of care plans being updated following changes in need.
