Patients described the hospice and staff being responsive to their individual needs. We observed this through detailed care plans which staff developed through conversations with patients and relatives, reflecting preferences and ensuring an inclusive approach to patient care.

Staff had an active approach to understanding diverse patient needs through regular interactions, emphasising on physical, family and spiritual needs.

The hospice worked closely with local health and social care partners to ensure a seamless transition of care for patients. The hospice worked with local faith leaders and LGBTQ+ groups as part of its outreach across the wider community.

The hospice was accessible for patients in the ward and community, with access to staff and a 24/7 advice line, ensuring continuous person-centred care. The use of electronic records ensured that teams had the most updated and accurate data regarding patient’s care plans and their physical and emotional needs to allow effective continuity of care. The hospice demonstrated an inclusive approach to ensuring services were accessible and served the diverse needs of the local community. The hospice had access to translation services and enabled translation additionally through multilingual staff and hand-held translation devices used on the ward and in the community. The hospice ensures an inclusive approach when making reasonable adjustments for patients with disabilities including information being provided in multiple formats and communication aids being available. The hospice delivered services which were tailored to suit the diverse and changing needs of their patients and community, including services for bereavement and people with learning difficulties. These services were organised with specific populations including a bereavement service and services for people with learning difficulties and neurodiverse people.
