Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. Delivering evidence-based care and treatment
Recognised tools were used to assess risks and support the development of care plans. However, tools used to assess the risk of weight loss and malnutrition were completed incorrectly. Information about some health conditions and the presentation of individuals could be developed further, to enhance the support provided and to ensure staff knowledge and practice is fully up to date with evidence-based good practice and standards.
