Safe
The service had enough staff to care for patients and keep them safe. Staff had training in key skills, understood how to protect patients from abuse, and managed safety well. However, within critical care we had concerns about the training compliance of staff in some key subjects including life support and safeguarding. There was a positive learning culture with staff managing incidents well. Learning from incidents was evident. Staff knew what incidents required reporting and how to report them. When things went wrong, staff were aware of the principles of being open and honest and where required, implemented the duty of candour. The processes in place supported staff to report and learning from incidents. Staff assessed risks to patients, acted on them and kept good care records. The service-controlled infection risk well. However, within outpatients clinical areas were not consistently visibly clean.
