Safe
The provider aimed to include staff when making plans for the service and look at where some improvements could be made. However, this could be documented more clearly to show how the provider monitored the learning culture of the service.
People’s records evidenced information such as risk assessments to inform staff on how to support the person safely. We saw there were sufficient numbers of staff, who had been appropriately recruited and trained to carry out their roles effectively.
Systems were in place for staff to know how to report any safeguarding concerns. Improvements needed to be made for the provider to report and record medicine errors and concerns to the local authority.

Learning culture
Staff knew the person well and knew how the person like their care provided. The person’s relative told us the person’s preferences were always considered.
Staff said that their opinions were sought and acted upon to improve the service. One member of staff said that “The management often seeks feedback from us to enhance the service.” They were able to provide examples of feedback being provided and actions that were taken following this.
All staff said that they knew who they could report any concerns, both internally and externally. None of the staff had ever had to report anything so they were unable to advise on actions taken by the provider.
The provider had developed an action plan and business development plan to record where improvements needed to be made and if there had been any lessons learnt following events in the service. From viewing these documents, it was not fully clear what areas of the service the provider took responsibility for.
Safe systems, pathways and transitions

Safeguarding
The person told us they felt safe. One relative said [when asked if they felt the person was safe with staff], “oh yes, very safe.”
Staff could list potential risks for the person, advised that risk assessments were in place and provided detail of potential risk. One staff member said that, “The risks differ for each [person]. However, risk assessments for our client [person] are in place to guide me.” Staff knew to report any concerns to both management and externally, including the local authority. Staff all said that they would be comfortable to report any poor practice witnessed.
There was a safeguarding policy and procedure in place to inform staff on how to report any safeguarding concerns.
Systems were in place to allow the provider record and report concerns to the appropriate authorities.
Involving people to manage risks
We asked a relative if they had been involved in the creation of risk assessments for the person and had the person’s risks been explained to them. They told us they had not.
Staff all said that they had the information they needed for the person using the service to carry out their tasks. They spoke about the use of care plans to show, “What they [peron] like and what they don’t like.” One staff member said these are always updated and the information on them is clear. Risk assessments have been mentioned by staff and they were able to list a number of possible risks that could occur.
Risk assessments were in place to help staff manage risks the person experienced. These risks included choking, mobility and skin damage. Staff were provided with guidance on how to manage risks and keep the person safe.
Safe environments

Safe and effective staffing
A relative told us there were sufficient staff to effectively support them.
Staff stated that they received online and face to face training when they joined the company. Staff also said they received an orientation to introduce them to the role and to cover various aspects of their job and the company policies. Staff have stated that training is ongoing, with in-person sessions arranged. One staff member said, “I do receive spot checks and one-on-one supervisions to ensure that I am providing quality care and to offer support and feedback as needed.”
Staff were effectively deployed to allow people to be safely supported. The provider had robust recruitment procedures in place to ensure only suitable, qualified staff were recruited. This included background and work history checks along with police criminal background checks.
Staff files were audited to ensure information was collated and checked for staff working in the service.
