We reviewed systems, pathways and transitions within the service and how the service was involving people to manage risk. Systems were in place and effectively used to ensure people safely transitioned into and out of the service. Staff worked closely with people to develop and complete their risk assessments, which considered plans for them unexpectedly leaving treatment and deterioration in their mental health. Medicines management systems had improved following the last inspection and up-to-date GP summaries were routinely collected. Medical decisions were clearly documented and handling, dispensing and documentation of medicines were robust. This meant all breaches of regulation identified at the previous inspection were addressed effectively.

Safe systems, pathways and transitions

We reviewed care records which evidenced people’s voice and wishes were considered and included in their care pathway. People had opportunities to feedback through keywork sessions about their experiences as they progressed through treatment.

Staff and leaders understood people’s needs and associated risks. As people’s care and treatment progressed staff reviewed their care needs to ensure the service continued to be effectively able to provide the support required.

Feedback from commissioning services in other areas of England shared positive feedback of the service following admissions to the service.

The service had processes in place to ensure people’s care needs and risks were considered to support safe transition into and out of the service. Systems and processes to ensure people’s needs and risks were captured prior to admission were in place. Leaders and staff worked in collaboration with people to complete assessments and actions plans which were reviewed regularly and updated as people’s needs changed.

Involving people to manage risks

We reviewed risk management plans for people using the service and found their views and wishes around safely managing risks relevant to them were captured.

Staff were knowledgeable about people’s care needs and associated risks. Staff told us how risk management plans were collaboratively undertaken with people to promote positive risk taking and Staff and leaders were able to explain how they involved people to manage risks. Specific members of staff collaborated with people receiving treatment to ensure inclusion in their care pathway.

Processes were in place to ensure people’s needs and risks were identified and documented prior to their admission. This helped to ensure people were supported by the correct service.

Medicines optimisation

Documentation we reviewed evidenced people were informed about the medicines required for their treatment. People had the opportunity to share their experiences regarding side effects of medicines upon their physical health and emotional wellbeing. This meant changes to medicines could be promptly made which improved people’s outcomes and experiences.

Staff and leaders told us they were using the new digital systems which supported them to manage and administer people’s medicines safely.

We observed medicines were managed effectively, and processes in place supported best practice.

The newly introduced digital system was in place and being effectively used by staff. The system assisted staff in reconciliation, administration and documentation of medicines. A process was in place to ensure up-to-date GP summaries were sought prior to admission. This meant the service could be assured appropriate medicines were prescribed correctly to people during their treatment.
