Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. Incidents and accidents involving people were recorded and these included any action taken by staff in response. These incidents were routinely reviewed to determine potential causes and to identify any actions they needed to take to reduce the likelihood of similar incidents reoccurring. Learning was shared and discussed with staff. There was evidence that the provider learnt lessons following any incidents that occurred. This included updating any risk assessments and associated records such as eating and drinking plans.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. Pre-admission assessments were completed when referrals for new placements were received. The registered manager confirmed these were used to help staff develop care plans for people. The registered manager confirmed they were working with health and social care partners to move a person to another placement, this was confirmed by the family member.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. People told us they felt safe. One person said, “Yes, I feel safe. This is my home.” Staff were aware of safeguarding reporting procedures and how to spot potential signs of abuse or harm. One member of staff told us, “Safeguarding is protecting and looking after the residents.” Training records showed staff had received up to date training in safeguarding adults. The registered manager understood his legal responsibility to immediately report any safeguarding incidents or concerns to the relevant health and social care partners .

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. Staff were aware of the risks people might face and the steps they needed to take to prevent or safely manage them. Staff told us they always followed people’s risk assessments to enable people to take reasonable and acceptable risks.

Safe environments

Score: 3

The service detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. The service had worked to support access to a bespoke and specialised living space for a person, designed to promote their safety, independence and privacy. People’s homes were well maintained and the environment was safe. Regular maintenance checks such as fire safety and fire evacuation drills were completed. There were personal emergency evacuation plans in place to help staff evacuate people in an emergency.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. People told us there were always enough staff available to support them. The registered manager said they regularly reviewed staffing levels to ensure there were always enough suitably experienced and trained staff on duty to meet people’s needs. Staff received relevant training to support them in their roles, including Oliver McGowan training. The Oliver McGowan Mandatory Training on Learning Disability and Autism is the government's preferred and recommended training for health and social care staff. Training records also showed staff had received training in first aid and basic life support which included Cardiopulmonary resuscitation (CPR). Staff told us about they received regular training and good support and supervision as part of their job roles.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. We observed people’s homes to be clean. Staff carried out cleaning and maintenance checks.

Medicines optimisation

Score: 3

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened. People told us staff supported them to take their prescribed medicines as and when they should. Staff told us they had received medicines training and their competency to continue managing medicines safely was routinely assessed. This was confirmed in the records that we saw.
