Responsive
We assessed a total of 2 quality statements from this key question. We have combined the scores for these areas with scores based on the rating from the last inspection, which was good. Our rating for this key question is Good. Observations of care showed staff provided care in a person-centred way and responsively accommodated individual needs. However, we did see some inconsistencies in case note documentation. Staff had a solid understanding of equality, diversity, inclusion and human rights and were proactive in preventing discrimination.

Person-centred Care
Carers told us that they were involved in the care planning for their relatives and were regularly invited to meetings regarding the care and treatment. Wards held carer afternoon tea sessions where carers could meet their relative and other carers and discuss the service. Patients had no specific feedback on this area.
Staff spoke to were aware of each patient's specific needs and the level of care and treatment required. Staff and managers had a good knowledge of the additional services they could utilise to enhance a patient's care and treatment. Staff informed us that they could provide information in different formats if required. Staff on the wards had good relationships with the palliative care team and third sector organisations to provide person centred and holistic palliative care on the ward if the family preferred this.
Observations showed that staff knew patients well and were able to provide treatment in a person-centred way. However, on our observation of the night shift we did not see person centred care, staff were busy and did not attend to people's needs in a timely manner.
The majority of care records demonstrated an holistic and person centred approach and looked at the whole person. We found evidence of physical health referrals and use of additional services. However, there were inconsistencies in the completion of aspects of case notes, most notable was risk and safety plans.

Equity in experiences and outcomes
People had no specific feedback on this area.
Staff clearly held a good understanding of equality and diversity guidelines. Staff also felt confident raising concerns. They also had good working knowledge of Mental Health Act /Mental Capacity Act and patients' rights. Access to beds was managed through a central bed management team, they allocated beds following a review of the acuity of patients and appropriateness of the placement by a multidisciplinary team.
Processes were in place to protect equality, diversity, inclusion and human rights. Wards were accessible for wheelchair users with widened doors and access to lifts. Review of patients records demonstrated Mental Health Act code of practice was followed and people’s right were well protected.
